Physical Therapist (PT) – Concierge Healthcare & Sports Performance

Northbrook, IL | Full-Time | Outpatient Orthopedics & Preventive Health

Salary + Performance Bonus Potential

Are you a Physical Therapist who loves orthopedic rehabilitation but also has a passion for fitness, performance, and helping patients achieve long-term health?

We are recruiting for an exciting opportunity with a growing concierge-style healthcare practice that delivers personalized, white-glove service in a collaborative, interdisciplinary setting. This unique practice goes beyond traditional outpatient physical therapy by combining orthopedic rehabilitation, sports performance, preventive medicine, and longevity-focused care.

With an emphasis on one-on-one treatment, exceptional patient experiences, and quality over quantity, this position offers the opportunity to build meaningful patient relationships, work alongside physicians and other healthcare professionals, and play an important role in launching a new Northbrook location.

Why You'll Love This Opportunity

  • Competitive Salary: 85,000 - 95,000 annually, very negotiable depending on experience, with potential for performance-based bonuses

  • Generous PTO: 20 days annually, accrued throughout the year, plus paid holidays

  • Excellent Benefits: Medical, dental, vision, life insurance, and short- and long-term disability coverage

  • 401(k): Retirement plan with up to a 3% employer match

  • True One-on-One Care: Dedicated 60-minute physical therapy sessions in a low-volume, cash-pay setting

  • Interdisciplinary Collaboration: Work closely with physicians, registered dietitians, and other healthcare professionals

  • Professional Development: $500 annually toward continuing education and specialty certifications, plus paid license renewals

  • Clinical Autonomy: Take ownership of your patient care and help establish physical therapy services at a new location

About the Opportunity

This is not your typical outpatient physical therapy position! The practice combines orthopedic rehabilitation, sports performance, preventive medicine, and longevity-focused healthcare through a personalized, concierge-style model.

You'll divide your clinical responsibilities between two areas:

Executive Health & Performance Assessments (Approximately 50%)

Serve as the movement and exercise specialist within an interdisciplinary team, helping patients better understand and improve their overall physical health. Assessments incorporate strength, mobility, cardiovascular fitness, body composition, and exercise recommendations to support long-term wellness and injury prevention.

One-on-One Outpatient Physical Therapy (Approximately 50%)

Provide individualized orthopedic rehabilitation, manual therapy, exercise prescription, and return-to-activity programming. You'll work with motivated, health-conscious patients in a setting that prioritizes quality over quantity.

The clinic features private treatment rooms and a fully equipped fitness space with strength training equipment, free weights, cardio machines, and rehabilitation tools.

Schedule & Location

  • Full-Time: Approximately 40 hours per week, Monday–Friday

  • Typical clinic hours are approximately 7:30 AM–5:30 PM, with some scheduling flexibility for private PT appointments

  • Expected start in January 2027 at a Chicago-area location for onboarding and training

  • Transition to the new Northbrook clinic beginning in March 2027, with the goal of becoming the primary on-site Physical Therapist

You'll work independently at the new location while receiving support and clinical guidance from an experienced Physical Therapy leader. No management experience is required.

Qualifications

Required:

  • Doctorate or Master's degree in Physical Therapy

  • Strong orthopedic assessment, manual therapy, and exercise prescription skills

  • Experience treating a variety of musculoskeletal and postoperative conditions

  • Understanding of strength, conditioning, recovery, and injury prevention

  • Confidence working independently and communicating with patients

  • Passion for helping patients transition from rehabilitation to performance and lifelong wellness

Preferred, but Not Required:

  • Experience in sports medicine, return-to-sport, running rehab, or strength & conditioning

  • Certifications such as OCS, CSCS, COMT, dry needling, or blood flow restriction training

  • Experience working with active adults, athletes, or direct-access patients

  • Additional clinical specialties or interests are welcome!
